Successful management of placenta percreta by cesarean hysterectomy with transverse uterine fundal incision

چکیده
Placenta accreta presents one of the highest risks to pregnancy, and its more severe variant, placenta percreta, is particularly risky. The incidence of both conditions is increasing. Placenta percreta requires a cesarean hysterectomy for management, but the challenges associated with this surgery often result in severe obstetric hemorrhaging and high rates of maternal morbidity. Several recent obstetric studies have reported on the usefulness of the transverse uterine fundal incision for the management of placenta accreta and its variants. However, these reports included only a few cases of placenta percreta. Here we present a case of placenta percreta covering the anterior uterine wall that was successfully managed using a transverse fundal incision, which avoided incising the placenta at delivery and thus reduced maternal blood loss. After delivery, the patient underwent a total abdominal hysterectomy without the need for a blood transfusion. We conclude that a transverse uterine fundal incision can be very useful for the management of placenta percreta of the anterior uterine wall.

Placenta percreta with a vaginal fistula after successful management by uterine transverse fundal incision and subsequent cesarean hysterectomy
Placenta previa presents a highest risk to pregnancy, and placenta accreta is the most serious. Placenta accreta requires cesarean delivery and often results in massive obstetric hemorrhage and higher maternal morbidity. Challenges associated with cesarean delivery techniques may contribute to increased maternal blood loss and morbidity rates. Several recent obstetric studies reported the usefu...
متن کاملPlacenta percreta managed by transverse uterine fundal incision with retrograde cesarean hysterectomy: a novel surgical approach
KEY CLINICAL MESSAGE Placenta percreta (with bladder invasion) is a rare obstetric condition with the risk of massive intraoperative hemorrhage. In these cases, the combination of a transverse uterine fundal incision and retrograde cesarean hysterectomy could be useful to minimize maternal hemorrhage and avoid severe bladder injury.

AIM To assess the usefulness of a new method for cesarean section (CS) that is comprised of a transverse incision into the uterine fundus, developed for women with placentas covering the entire anterior uterine wall, and introduced in September 2006. MATERIAL AND METHODS Review of medical records of 12 and 29 women who underwent CS by the new and conventional methods, respectively, for placen...
متن کاملConservative management of placenta previa percreta by leaving placental tissue in situ with arterial ligation and adjuvant methotrexate therapy.
Placenta percreta is one of the life-threatening conditions in modern obstetrics. The rising caesarean section rate means rising placenta percreta rate. Treatment strategies range from a caesarean hysterectomy to leaving the placenta in situ with or without internal iliac artery ligation/uterine artery embolisation and/or methotrexate therapy. We describe a case of placenta previa percreta whic...

BACKGROUND Placenta percreta is a rare obstetric condition associated with the risk of massive intraoperative hemorrhage. Recently, conservative management of placenta percreta has been performed to reduce maternal morbidity. However, various complications have been reported during such management. Only a few cases of asymptomatic disseminated intravascular coagulation (DIC) or fever without in...
